PRIVATE-PRESCRIPTION / REFERRAL
You can now record private-prescriptions and transfers via our prescription telephone or use our contact form. We are also happy to accept your inquiries by email and telephone.
Information about the E-Prescription
Please note that you are still required to present your health insurance card as proof of insurance at our practice once every quarter.
If your card has already been scanned this quarter and you have previously received the medication from us, you can request an e-prescription using our form, which will be available on your insurance card the next business day from 3 p.m., (Monday to Friday).
If your card has not yet been scanned or you require a referral, you can use our form for pre-ordering. You can then collect both the referral and the prescription at our practice the next business day from 3 p.m., (Monday to Friday), after presenting your insurance card.
